404: Fisheye is not enabled referer=null

Symptoms
When trying to view a revision from a Crucible review or a Source tab in JIRA, the following message is presented on screen:

Received status code 404 (Not Found)
And in the Fisheye/Crucible logs:

Cause
Only a Crucible license is present. There is no license for Fisheye, hence it is not available.
Resolution
Generate a Fisheye license at http://my.atlassian.com and apply to Fisheye/Crucible under Administration > System Settings > System Info > Edit License
